Volunteer/Adjunct online form. Volunteer Identification Pre-Assessment.
- It is assumed that you are filing out this form because you have received consent from one of our faculty members to join their lab as a volunteer or they have agreed to authorize your appointment as an adjunct professor.
- If you will be enrolled in classes at SDSU during the period that you wish to volunteer, you do not need to submit this form.
- Please note, the data collected in this file is for departmental use only.
